Your physical therapist will use a comprehensive approach in determining exactly what you will need to obtain the maximum benefit from your rehab. First, your therapist will do a thorough evaluation and then develop a customized plan of care that addresses all of these specific findings and goals. Sometimes a home exercise program is developed and given to you to perform between sessions.

In Oregon you can see a physical therapist without a referral from a physician. However, your insurance may require a referral from a physician to cover your expenses. To find out, call the number on the back of your insurance card and ask if a referral is necessary for coverage of physical therapy.

Yes! You can choose your physical therapy provider even if you have a prescription for a different physical therapist. Bring your prescription with you to your first appointment and we will take care of the rest!

We participate with almost all health insurance and auto insurance programs, including Medicare and Worker’s Compensation. Please call any location to find how we can handle your insurance coverage.

We also participate in the Volunteers in Medicine for the uninsured.

  • Be prepared to pay any co-pays at the time of service. We accept Visa, Mastercard, American Express, personal checks and cash.

Expedite your initial visit by downloading and filling out our Patient Registration Forms ahead of time. Please bring a copy to your first visit.

Wear comfortable, loose clothing and tennis shoes. We have a changing room available for your convenience. Please be conscious of wearing loose clothing around the area specific to your injury. Example: neck or shoulder injury, wear a tank top, knee or hip injury, wear shorts, etc.

If you are coming for aquatic therapy please come already wearing a one piece bathing suit and don’t forget a towel.

Your first visit includes the comprehensive evaluation and takes 60-90 minutes. Sessions thereafter are determined by your condition, approximately 45-60 minutes. Your physical therapist will develop an individually customized plan of care, so each program will be specific to your needs.

Your Plan of Care will be constructed based on the recommendation of your doctor and your physical therapist. The average physical therapy schedule is 1-3 times per week. Your schedule will depend on the degree of injury/dysfunction and your available time. Bring your calendar to your first appointment so you can schedule your appointments as far in advance as possible, this will make it easier to choose times that are the most convenient for you.

Your Rebound physical therapist is in regular communication with your doctor, especially if there are any complications.

Rebound Physical Therapy adheres to all the requirements of the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA.

All patients are provided notice of Rebound Physical privacy practices. Patient records are considered protected health information and strict confidentiality is maintained in compliance with HIPAA rules. Records will be released for treatment to referring physicians and for health insurance purposes with the consent of the patient.
